  5. Fibrotic Rewiring of CD8+ T-cells by Doxorubicin Drives Pathological Remodeling of Cardiac Extracellular Matrix | JACC: CardioOncology

Dr. Caitlin O'Meara discusses a JACC: CardioOncology study showing that doxorubicin reprograms CD8+ T cells into matrix-remodeling T cells (mxT-cells) that produce lysyl oxidase (LOX) and stiffen the cardiac extracellular matrix through HIF1α-P53-TGFβ signaling. She explains how this new immune mechanism of anthracycline cardiotoxicity could lead to therapies that prevent cardiac fibrosis, cardiomyopathy, and heart failure in patients treated with chemotherapy.

  6. High-Sensitivity Cardiac Troponin T and I Discordance in Patients Receiving Immune Checkpoint Inhibitors | JACC: CardioOncology

Dr. David Austin discusses a JACC: CardioOncology study showing that high-sensitivity cardiac troponin T was elevated in 37.9% of patients starting immune checkpoint inhibitor (ICI) therapy, compared with only 1.8% for high-sensitivity troponin I, a discordance linked to older age, male sex, and kidney dysfunction. He explains what this means for troponin assay selection, baseline testing, and ICI myocarditis surveillance, helping cardiologists and oncologists avoid unnecessary cancer treatment interruptions and immunosuppression.

She explains what these inflammatory cell profiles mean for cardio-oncology care, from immunosuppression and ICI rechallenge to balancing cardiac safety with effective cancer immunotherapy.

  20. Multimodality Imaging and Pace-Mapping Patterns in Patients With Cardiac Sarcoidosis and Ongoing Inflammation Undergoing Ventricular Tachycardia Ablation | JACC: Clinical Electrophysiology

In this episode, Frank Marchlinski, MD, Deputy Editor of JACC: Clinical Electrophysiology, discusses a study of ventricular tachycardia ablation in patients with active cardiac sarcoidosis. Using integrated FDG-PET and cardiac MRI imaging, the investigators found that VT exit sites were often located in areas of active inflammation, while more proximal circuit sites were associated with scar. In this episode, Frank Marchlinski, MD, Deputy Editor of JACC: Clinical Electrophysiology, discusses a study evaluating how CT-defined myocardial ridges and functional mapping can be combined to identify critical ablation targets in post-infarction ventricular tachycardia. The investigators found that VT target sites frequently localized to CT-defined ridges, particularly those associated with primary deceleration zones, suggesting that functional mapping can help distinguish active arrhythmogenic ridges from inactive ones and support more targeted VT ablation.
